The Town of Silverthorne and the Silverthorne Recreation Center welcomes everyone in the community to participate in programs.

If you or your child require accommodations, or if your child has a medical condition that may require intervention (such as asthma, seizure disorders, life-threatening allergies, diabetes, etc.), please complete the relevant forms below and submit them to the Recreation Center front desk or email them to RecAccommodations@silverthorne.org at least two weeks prior to program attendance. Requests made within two weeks of the program start date may not be able to be accommodated.  For your child’s safety, requests made the day of program start will not be accommodated. 

• Colorado Asthma Care Plan (Use this form for inhalers)
• Standard Health Plan for Allergy-Anaphylaxis (Use this form for epinephrine)
• Seizure Emergency Care Plan (Use this form for seizure care)
• Medication Authorization Form (Not for inhaler, epinephrine, seizure medications or diabetes care)
• Needs Questionnaire (Use for all other accommodation requests)
